About The Position

Signify Health, part of CVS Health, is seeking a full-time Nurse Practitioner or Physician Assistant to conduct in-home health evaluations and help bridge gaps in care. Clinicians visit health plan members in their homes, build trusted relationships, and provide flexible, comprehensive, personalized evaluations. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, time to answer health questions, and additional diagnostic tests when ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medication, order lab tests, or change a member’s current treatment regimen. The position involves local travel and travel within licensed states as needed, as well as virtual visits when needed. The primary workplace is members’ private residences, where conditions vary and may include household pets, secondhand smoke, and other airborne allergens or irritants. Candidates must be able to perform their duties comfortably and safely in these environments. The role is full-time, with 40 anticipated weekly hours. The stated pay range is $87,035 to $187,460. The actual offer depends on factors including experience, education, geography, and other relevant factors.

Requirements

Active, unrestricted license(s) in the coverage area(s).
Completion of 2,080 hours of post-graduate collaborative practice hours as an NP or PA in an integrated clinical care setting.
Filing of the appropriate attestation with the state of Minnesota.
Board certification.
Ability to comfortably and safely perform duties in home environments that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities

Conduct in-home health evaluations for health plan members, including medication and medical history reviews and physical evaluations.
