About The Position

Signify Health, part of CVS Health, is seeking a part-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations to health plan members in St. Cloud, Minnesota. The role focuses on engaging with people in their homes, building trusted relationships, and helping bridge gaps in care. Visits typically last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medicine, order lab tests, or alter members’ current treatment regimens.

The primary workplace is members’ private residences. Home environments vary and may include household pets, secondhand smoke, and other airborne allergens or irritants. The position may also involve local and in-state travel and virtual visits as needed. Requirements

• Active, unrestricted license(s) in coverage area(s).
• Board certification.

Responsibilities

• Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
• Perform additional diagnostic tests when ordered by a member’s health plan, such as diabetic eye exams, spirometry, and bone density screening.
• Use an iPad and the clinical workflow for each evaluation.
• Build trusted relationships with members and help connect them to appropriate care.
• Travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
• Provide Signify Health services as indicated, including Diagnostic & Preventive Services products.
