About The Position

Signify Health, part of CVS Health, is seeking a part-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations to health plan members. The purpose of the visits is to build trusted relationships, help members understand their health, and bridge gaps in care. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and, when ordered by the member’s health plan, additional diagnostic tests such as diabetic eye exams or spirometry. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medicine, order lab tests, or change a member’s current treatment regimen. The clinician’s primary workplace is members’ private residences. Home environments vary and may include household pets, secondhand smoke, and other airborne allergens or irritants. The role may also include local travel, travel within licensed states as needed, and virtual visits. The position is part time, with anticipated weekly hours of 29.5.

Requirements

Active, unrestricted license(s) in the coverage area(s).
Board certification.

Responsibilities

Conduct in-home health evaluations and build trusted relationships with health plan members.
Review members’ medication and medical histories and perform physical evaluations.
Perform additional diagnostic and preventive services as indicated, such as spirometry, diabetic eye exams, and bone density screenings.
Conduct virtual visits as needed.
Travel locally and within licensed states as needed.
Provide health education and answer members’ questions during visits.
Fulfill the requirements of the specified clinician role and provide Signify Health services as indicated.
