About The Position

Signify Health, part of CVS Health, is seeking a part-time Nurse Practitioner or Physician Assistant to provide In-Home Health Evaluations in members’ homes and help bridge gaps in care. The role focuses on building trusted relationships and providing flexible, comprehensive, personalized health evaluations. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the person’s health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medicine, order lab tests, or alter a person’s current treatment regimen.

The primary workplace is the private residence of health plan members. Home environments vary and may include household pets, secondhand smoke, and other potential airborne allergens or irritants. The position also involves local travel and travel within licensed states as needed, and virtual visits as needed.

Requirements

• Active, unrestricted license(s) in coverage area(s).
• Board certification.

Responsibilities

• Conduct In-Home Health Evaluations, including medication and medical history reviews and physical evaluations.
• Build trusted relationships with health plan members and help connect them to care suited to their needs.
• Use an iPad and clinical workflow to complete evaluations.
• Travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
• Provide Signify Health services as indicated, including Diagnostic & Preventive Services such as spirometry, diabetic eye exams, and bone density screening.
• Fulfill the requirements of the specified clinician role.
