In-Home Nurse Practitioner or Physician Assistant Part Time - Medford
About The Position
Signify Health, part of CVS Health, is seeking a part-time Nurse Practitioner or Physician Assistant to conduct In-Home Health Evaluations for health plan members. The clinician visits members in their homes to build trusted relationships, help identify gaps in care, and connect people with care that meets their needs. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and any additional diagnostic tests ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medication, order lab tests, or change a member’s current treatment regimen. The position involves local travel and travel within licensed states as needed, with virtual visits also conducted as needed. The primary work environment is members’ private residences, which may contain household pets, secondhand smoke, or other airborne allergens or irritants. Requirements
- Active, unrestricted license(s) in the coverage area(s).
- Board certification.

Responsibilities
- Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
- Perform additional diagnostic tests when ordered by a member’s health plan, including services such as spirometry, diabetic eye exams, and bone density screening.
- Use an iPad and the clinical workflow for each evaluation.
- Help members understand their health and connect them with appropriate care.
- Travel locally and within licensed states as needed.
- Conduct virtual visits as needed.
- Provide Signify Health services as indicated, including Diagnostic & Preventive Services products.
- Fulfill the requirements of the specified clinician role.
