About The Position

Signify Health, part of CVS Health, is seeking a part-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations and help bridge gaps in care. Clinicians engage with health plan members in their homes, build trusted relationships, and provide flexible, comprehensive, and personalized health evaluations. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medication, order lab tests, or change a member’s current treatment regimen.

The primary workplace is the private residence of health plan members. Home environments vary and may include household pets, secondhand smoke, and other airborne allergens or irritants. Clinicians must be able to perform their duties comfortably and safely in these environments. Local and in-state travel may be needed, as well as virtual visits.

This part-time position anticipates 29.5 weekly hours. The typical pay range is $46.03–$99.14. The actual offer depends on factors including experience, education, geography, and other relevant factors. Requirements

• Active, unrestricted license(s) in the coverage area(s).
• Board certification.
• Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities

• Conduct in-home health evaluations for health plan members, including medication and medical history reviews and physical evaluations.
• Perform additional diagnostic tests when ordered by a member’s health plan, such as diabetic eye exams, spirometry, or bone density screening.
• Use an iPad and the clinical workflow for each evaluation.
• Build trusted relationships with members and help connect them to appropriate care for their needs.
• Provide Signify Health services as indicated, including Diagnostic & Preventive Services products.
• Travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
• Fulfill the requirements of the specified clinician role.
