About The Position

Signify Health, part of CVS Health, is seeking a part-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations in Lansing. Clinicians engage with health plan members in their homes, build trusted relationships, and help bridge gaps in care by connecting people with care suited to their needs.

Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and—when ordered by the member’s health plan—additional diagnostic tests such as diabetic eye exams or spirometry. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medication, order lab tests, or change members’ current treatment regimens. Virtual visits may also be conducted as needed.

The primary workplace is members’ private residences. Home environments vary and may include household pets, secondhand smoke, or other airborne allergens or irritants. Clinicians must be able to perform their duties comfortably and safely in these settings. Local travel and travel within licensed states may be needed.

Requirements

• Active, unrestricted license(s) in the coverage area(s).
• Board certification.
• Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, or other airborne allergens or irritants.

Responsibilities

• Provide in-home health evaluations and engage members in the comfort of their homes.
• Review members’ medications and medical histories and conduct physical evaluations.
• Perform additional diagnostic tests, such as diabetic eye exams or spirometry, when ordered by the member’s health plan.
• Use an iPad and the clinical workflow for each evaluation.
• Conduct virtual visits as needed.
• Provide Signify Health services as indicated, including Diagnostic & Preventive Services products.
• Travel locally and within licensed states as needed.
• Help connect members to appropriate care for their individual needs.
