About The Position

Signify Health, part of CVS Health, is seeking a part-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations and help bridge gaps in care. Clinicians visit health plan members in their homes, build trusted relationships, and provide flexible, comprehensive, personalized evaluations. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan, such as diabetic eye exams or spirometry. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medication, order lab tests, or change a member’s current treatment regimen.

The primary workplace is the private residence of health plan members. Home environments vary and may include household pets, secondhand smoke, and other potential airborne allergens or irritants. Clinicians must be able to perform their duties comfortably and safely in these conditions. The role may also involve local travel, travel within licensed states as needed, and virtual visits.

The position is part-time, with anticipated weekly hours of 29.5. The typical pay range is $41.85–$90.13 per hour; the actual offer depends on experience, education, geography, and other relevant factors. Responsibilities

• Provide in-home health evaluations to health plan members.
• Conduct visits of about 45 minutes, including medication and medical history reviews and physical evaluations.
• Provide additional diagnostic tests when indicated by the member’s health plan, including services such as spirometry, diabetic eye exams, and bone density screening.
• Use an iPad and the clinical workflow for each evaluation.
• Build trusted relationships and help members connect with appropriate care.
• Provide all Signify Health services indicated, including Diagnostic & Preventive Services products.
• Travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
• Fulfill the requirements of the clinician role as a Signify Health colleague.
