About The Position

Signify Health, part of CVS Health, is seeking a part-time Nurse Practitioner or Physician Assistant to provide In-Home Health Evaluations to health plan members in their homes. The role focuses on building trusted relationships, helping people understand their health, and connecting them with appropriate care. Visits typically last about 45 minutes and include a medication and medical history review, a physical evaluation, and any additional diagnostic tests ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medication, order lab tests, or change a member’s current treatment regimen.

The primary workplace is members’ private residences. Home environments vary and may include household pets, secondhand smoke, and other airborne allergens or irritants; clinicians must be able to perform their duties comfortably and safely in these conditions. The role also involves local travel and travel within licensed states as needed, as well as virtual visits when needed. The position is part time, with 29.5 anticipated weekly hours. The stated pay range is $41.85–$90.13 per hour, with the actual offer dependent on factors including experience, education, and geography.

• Active, unrestricted license(s) in the coverage area(s).
• Board certification.

Responsibilities

• Conduct in-home health evaluations, typically lasting about 45 minutes.
• Review members’ medication and medical history and perform a physical evaluation.
• Perform additional diagnostic tests when ordered by a member’s health plan, such as diabetic eye exams, spirometry, or bone density screening.
• Use an iPad and the clinical workflow for each evaluation.
• Help connect members with care suited to their needs and contribute to improved health outcomes.
• Provide all Signify Health services indicated, including Diagnostic & Preventive Services products.
• Travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
• Fulfill the requirements of the specified clinician role.
