About The Position

Signify Health, part of CVS Health, is seeking a part-time Nurse Practitioner or Physician Assistant to conduct In-Home Health Evaluations for health plan members in the Kalamazoo area. The approximately 45-minute visit takes place in a member’s home and includes time for one-on-one connection and questions, a medication and medical history review, and a physical evaluation. Additional diagnostic tests may be provided when ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow. The role does not require prescribing medicine, ordering lab tests, or changing a member’s current treatment regimen. Clinicians may travel locally and within licensed states, conduct virtual visits as needed, and provide Signify Health services including diagnostic and preventive products such as spirometry, diabetic eye exams, and bone density screening. The primary workplace is members’ private residences, which may include household pets, secondhand smoke, or other airborne allergens or irritants. Requirements

Active, unrestricted license(s) in the coverage area(s).
Board certification.
