About The Position

Signify Health, part of CVS Health, is seeking a part-time Nurse Practitioner or Physician Assistant to provide In-Home Health Evaluations to health plan members. Clinicians engage with members in their homes, build trusted relationships, help bridge gaps in care, and connect people with care suited to their needs. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medicine, order lab tests, or change a member’s current treatment regimen.

The primary workplace is members’ private residences. Home environments vary and may include household pets, secondhand smoke, and other potential airborne allergens or irritants. The position may also involve local travel and travel within licensed states, as needed, and virtual visits as needed. The anticipated schedule is 29.5 hours per week.

Requirements

• Active, unrestricted license(s) in the coverage area(s).
• Board certification.
• Ability to comfortably and safely perform duties in home environments that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities

• Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
• Perform additional diagnostic tests when ordered by the member’s health plan, including tests such as diabetic eye exams and spirometry.
• Use an iPad and the clinical workflow for each evaluation.
• Build trusted relationships with members and help connect them with care suited to their needs.
• Travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
• Provide Signify Health services as indicated, including Diagnostic & Preventive Services such as spirometry, diabetic eye exams, and bone density screening.
• Work as a clinician colleague and fulfill the requirements of the specified role.
