About The Position

Signify Health, part of CVS Health, is seeking a part-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations and help bridge gaps in care. Clinicians engage with health plan members in their homes and provide flexible, comprehensive, and personalized evaluations. Visits last about 45 minutes and include time to connect with members and answer health questions, review medications and medical history, and conduct a physical evaluation. Additional diagnostic tests may be performed when ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow and are not required to prescribe medication, order lab tests, or change a member’s current treatment regimen.

The primary workplace is the private residence of health plan members. Home environments vary and may include household pets, secondhand smoke, and other airborne allergens or irritants. Clinicians must be able to perform their duties comfortably and safely in these environments. The role may also involve local travel and travel within licensed states, as needed, as well as virtual visits.

This part-time role is expected to involve 29.5 hours per week. Requirements

• Active, unrestricted license(s) in the coverage area(s).
• Board certification.
• Ability to comfortably and safely perform duties in home environments that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities

• Conduct in-home health evaluations for health plan members.
• Review members’ medications and medical histories and perform physical evaluations.
• Answer members’ health questions and help connect them with appropriate care.
• Perform additional diagnostic tests when ordered by the member’s health plan, including services such as diabetic eye exams, spirometry, and bone density screening.
• Use an iPad and the clinical workflow for each evaluation.
• Travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
• Provide Signify Health services as indicated, including Diagnostic & Preventive Services products.
