Requirements

Responsibilities

Conduct comprehensive occupational therapy evaluations and develop individualized treatment plans for residents.
Implement evidence-based OT interventions to improve activities of daily living, mobility, cognition, and safety.
Monitor progress, adjust goals, and document outcomes in the facility’s electronic health record.
Collaborate with an interdisciplinary team including nursing, physical therapy, speech-language pathology, and social work to support discharge planning and care transitions.
Provide education to residents and families on safety, adaptive equipment, and strategies to maintain function.
Ensure compliance with regulatory standards and facility policies while maintaining quality of care.
