About The Position
Under the supervision of the Transportation Coordinator, the Transportation Escort provides door-through-door transportation assistance for PACE participants traveling to the PACE Center, home, medical appointments, and other destinations. The Escort communicates while the van is in motion and reports observed changes in participants’ conditions. The role is part-time, Monday through Friday, for 30 hours per week.
Requirements
- Fluency in Hmong.
- High school diploma or equivalent.
- A minimum of 1–2 years of experience working with older adults, preferably in a PACE, adult day health, long-term care, or healthcare setting; training will be provided if this experience is not present.
- Reliable transportation, a valid driver’s license, and at least the state-required minimum liability auto insurance.
- Medical clearance for communicable diseases and up-to-date immunizations before direct participant contact.
Responsibilities
