About The Position

The Service Coordination Supervisor oversees the operational functions of the LTSS Service Coordination team using the AmeriHealth Caritas Pennsylvania Community Health Choices model of care. The role oversees day-to-day LTSS department operations, including development of Person-Centered Service Plans, service authorization and approval, and coordination of benefits for LTSS Participants. The supervisor also serves as an LTSS training subject matter expert, supports coordination with clinical and social services across the continuum of care, maintains operational processes and policies, and ensures quality and performance metrics are met.

Requirements

Bachelor's degree.
Pennsylvania-licensed RN, LSW, or LPC.
1 to 3 years of leadership and/or supervisory experience.
3 years of experience working in a social service or healthcare-related field.
2 years of experience working in Service Coordination with a Managed Care Organization.
Experience working with people with disabilities or seniors in need of LTSS.
Knowledge of the home and community-based service system and how to access and arrange services.
Ability to provide informed advocacy.
Ability to interact professionally with health care professionals.

Responsibilities

Oversee day-to-day operations of the LTSS department.
Oversee development of Person-Centered Service Plans.
Oversee authorization and approval of services and coordination of benefits for LTSS Participants.
Serve as a subject matter expert for LTSS training for internal care teams and external audiences.
Support coordination of LTSS with other clinical and social services across the continuum of care to enhance the quality of Participant care and services.
Maintain operational processes, policies, and procedures that support LTSS care delivery.
Ensure LTSS quality and performance metrics are consistently met as specified in the agreement.
