About The Position

Counterpart Health is hiring a Staff Product Manager to own the longitudinal patient record, a trusted view of care across settings that supports every Counterpart product. The role sets the record’s strategy and roadmap, balancing completeness, accuracy, timeliness, and investment across the needs of its many users and dependent products. The Product Manager will define consistent representations of care events—including dates, settings, providers, and facilities—and how medications, conditions, labs, and procedures connect to those events and to a patient’s history over time. They will establish clinically grounded rules for combining data from multiple sources while preserving traceability to the underlying evidence. The role also leads the creation of unified records of acute events, including hospitalizations, emergency department visits, and post-discharge care transitions. The shared record supports providers at the point of care as well as quality measurement, risk adjustment and RADV audits, payment integrity, and revenue operations. The role owns patient matching, deduplication, normalization, and clinical terminology mapping so that data from new sources can be incorporated consistently and improvements can be reused across products. It also leads adoption, including requirements, use-case-specific migration plans, and retirement of fragmented duplicate data versions. The Staff Product Manager leads the Longitudinal Patient Record pod and reports to the Director of Product, Clinical Data Products. They partner with engineering, clinical teams, and data consumers across the company. Success means the record becomes more complete, accurate, and timely; products and programs adopt the shared record; new customers and markets can reach a usable record with less one-off work; and downstream improvements are measurable. The role also establishes clear decision-making with clinical and engineering partners and ensures definitions are documented, adopted, and applied consistently.

Requirements

  • 8+ years of product management experience, including ownership of a data platform, data model, or shared data product used by multiple teams.
  • Experience setting product strategy for a platform with many internal consumers and making associated trade-offs.
  • Deep working knowledge of healthcare data and its clinical utility, including institutional and professional claims, C-CDA clinical documents, FHIR resources, ADT feeds, and lab data.
  • Ability to understand what different healthcare data sources cover and where their gaps are.
  • Experience defining canonical or master data such as patient, provider, facility, encounter, and clinical concepts, and driving adoption by other teams.
  • Ability and willingness to work directly with data by reviewing charts, investigating discrepancies, running analyses, and testing definitions against real records before release.
  • Experience using AI to accelerate data work and validating AI output.
  • Experience leading cross-functional projects through influence and driving delivery across engineering, clinical, and operations teams not directly managed.
  • Ability to demonstrate how an owned data improvement contributed to a business result, such as improved clinical outcomes, reduced duplication of care, or physician time savings.

Responsibilities

  • Set the vision and roadmap for the longitudinal patient record, identify high-value problems across dependent products and programs, and make trade-offs among completeness, accuracy, timeliness, and investment.
  • Define consistent representations of care events, including dates, settings, providers, and facilities, and connect medications, conditions, labs, and procedures to events and patient history over time.
  • Set clinically grounded rules for combining information from multiple sources and ensure facts can be traced to their supporting evidence.
  • Develop a unified record of hospitalizations, emergency department visits, and post-discharge care management using sources such as ADT notifications, prior authorization and admission notices, facility claims, medical records, discharge documentation, and discharge destinations.
  • Ensure the shared record supports point-of-care providers and programs in quality measurement, risk adjustment and RADV audits, payment integrity, and revenue operations.
  • Own patient matching, deduplication, normalization, and clinical terminology mapping to incorporate source data into a trusted patient record.
  • Lead adoption of canonical data through requirements, migration plans for each use case, and retirement of fragmented duplicate data versions.
  • Measure improvements in record completeness, accuracy, and timeliness and demonstrate their downstream impact.
  • Drive clear decision-making with clinical and engineering partners and ensure definitions are documented and consistently applied.
  • Lead the Longitudinal Patient Record pod and drive cross-functional delivery across engineering, clinical, and operations teams.
