About The Position

Brookhaven Hospice is seeking a patient-centered Licensed Master Social Worker (LMSW) to provide social work services to hospice patients and their families. The role focuses on counseling and guidance that support emotional well-being and coping capacity, while helping patients and loved ones experience dignity, peace, and connection during end-of-life care. The schedule is Monday through Friday, 8:00 a.m. to 4:30 p.m., with an on-call rotation. The coverage area includes Newbury Port, Burlington, Groton, Amesbury, Westford, Woburn, North Reading, and Methuen. A $5,000 sign-on bonus is offered. The listed average pay range is $34.00–$40.00 per hour; final compensation depends on relevant experience and the position’s responsibilities and is confirmed at the conclusion of the interview process.

Requirements

Master of Social Work degree from an accredited school of social work.
License as a Social Worker from the State Board of Social Work.
Current driver’s license and vehicle insurance, and access to a dependable vehicle.
At least one year of experience as a social worker in a healthcare setting.

Nice To Haves

At least one year of experience as a social worker in a hospice setting.
Experience in a long-term care or medical-surgical/acute-care setting.

Responsibilities

Assess patients’ psychosocial status and communicate it to the RN Case Manager and interdisciplinary group.
Evaluate medical needs, home situations, financial resources, and community support.
Implement individualized social work plans and collaborate with the Bereavement Coordinator.
Contribute to care plans, attend interdisciplinary meetings, and address psychosocial stresses.
Provide empathetic counseling to patients and families.
