About The Position

Provides a wide array of social work services to patients and families, including conducting psychosocial assessments, developing and coordinating discharge plans to ensure continuity of care, and providing supportive counseling to enhance coping and functioning. May be asked to resolve issues on all shifts, seven days a week. Reports to the Supervisor of Case Management/Director of Quality Case Management and performs other duties as assigned.

Requirements

• Three years of experience in clinical social work.
• Master of Science degree and licensure as an LSW.
• Advanced communication, conflict management, organizational, and planning skills.
• Basic computer literacy skills.

Nice To Haves

• Three years of experience in clinical social work.
• Master of Science degree with LMSW or LCSW preferred.
• LCSW or LMSW licensure preferred.

Responsibilities

• Conduct psychosocial assessments of patients and families of all ages to determine support and discharge-planning needs.
• Coordinate and collaborate on discharge plans with patients, families, and other healthcare providers to support smooth transitions between levels of care while maintaining an appropriate length of stay.
• Provide supportive counseling to individuals, families, and groups to address concerns and improve understanding, coping, and adjustment to illness or hospitalization.
• Maintain current knowledge of and accessibility to community resources, placement facilities, and financial agencies to meet patient and family needs.
• Share pertinent information with healthcare team members to develop comprehensive and appropriate discharge plans.
• Comply with policies that protect patients’ rights to considerate care, personal dignity, and respect for their cultural, psychosocial, and spiritual values and preferences.
• Complete assigned goals.
• May be asked to resolve issues on all shifts, seven days a week.
• Perform other duties as assigned.
