About The Position

Under the direction of the Clinical Supervisor or designee, the Social Worker provides services to help resolve patients’ social and emotional problems through psychosocial assessment and discharge planning. The role serves referred patients in inpatient medical, surgical, or pediatric units, or in an ambulatory care setting. The Social Worker maintains an active caseload and provides assessment, treatment, crisis intervention, and other appropriate services to patients and families, while collaborating with interdisciplinary teams and documenting services as required.

Requirements

• Sequence of study or experience in clinical practice in accordance with current New York State standards for licensure.
• Previous experience in the appropriate inpatient or outpatient setting, as indicated.
• New York State Licensed Social Worker (legally required and a policy requirement).
• Master’s in Social Work (required).
• Computer literacy (required).

Responsibilities

• Establish and maintain positive, professional, courteous, and appropriate relationships with patients, visitors, and employees, supporting a positive public perception of Bronx-Lebanon Hospital Center.
• Carry an active caseload of patients and families in accordance with departmental policies.
• Provide social work services to patients and families, including biopsychosocial and cultural assessment and treatment, and short- or long-term crisis intervention using individual, family, or other modalities as indicated.
• Collaborate with the interdisciplinary team in providing patient care.
• Provide reports, record documentation, and statistics as required.
• May serve as a liaison within the department and hospital, and with community agencies.
• May be delegated responsibility for in-service training and teaching social work students.
• Represent the Social Work Department at job-related community meetings and conferences.
• Perform other appropriate activities to support patient services and the functioning of the Social Work Program.
