About The Position

The Adult Day Care Center at Waters Place in the Bronx is seeking a full-time Social Worker to join its team. The Social Worker will work as part of an interdisciplinary team serving clients in the Dementia Care Enhanced Memory Impaired program. The role includes completing comprehensive MDS and psychosocial assessments, coordinating care planning and documentation, and facilitating team and family meetings. It also involves educating others about older adults’ rights and well-being and the needs of the vulnerable elderly population, with extensive work with family members and friends.

Requirements

• Master of Social Work degree from an accredited School of Social Work.
• Valid New York State license in Social Work (LMSW).
• Five years of dementia care experience.
• Solid written, communication, and organizational skills.

Nice To Haves

• Bilingual in English and Spanish.
• Certified in Dementia Care.
• Experience in an adult day care center.
