About The Position
The Adult Day Care Center located at Waters Place in the Bronx is seeking a full-time Social Worker to join its team. The Social Worker will work with clients in the Dementia Care and Enhanced Memory Impaired programs, collaborating with an interdisciplinary team to provide comprehensive MDS and psychosocial assessments. The role also involves coordinating care planning and documentation, facilitating team and family meetings, and educating others about older adults’ rights, well-being, and the needs of vulnerable older adults. The position includes extensive work with family members and friends.
Responsibilities
- Work as part of an interdisciplinary team to provide comprehensive MDS and psychosocial assessments for clients in the Dementia Care and Enhanced Memory Impaired programs.
- Coordinate and facilitate care planning and completion of all documentation.
- Facilitate team and family meetings.
- Provide education on older adults’ rights and well-being and the needs of the vulnerable elderly population.
- Work extensively with family members and friends.
