About The Position

Provides social work services to patients and families, including psychosocial assessments, discharge planning to support continuity of care, and supportive counseling to enhance coping and functioning. May be asked to resolve issues on all shifts, seven days a week. Reports to the Supervisor of Case Management/Director of Quality Case Management and performs other duties as assigned.

Requirements

• At least 3 years of experience in clinical social work.
• Master of Science degree and LSW licensure.
• Advanced communication, conflict-management, organizational, and planning skills.
• Basic computer literacy skills.
• May be asked to resolve issues across all shifts, seven days a week.

Nice To Haves

• LMSW or LCSW preferred.
• LCSW or LMSW licensure preferred.
• 3 years of experience in clinical social work is listed as preferred/desirable.

Responsibilities

• Conduct psychosocial assessments of patients and families of all ages to determine support and discharge-planning needs.
• Coordinate and collaborate on discharge plans with patients, families, and other healthcare providers to support smooth transitions between levels of care while maintaining an appropriate length of stay.
• Provide supportive counseling to individuals, families, and groups to address identified concerns and enhance understanding, coping, and adjustment to illness or hospitalization.
• Maintain current knowledge of and access to community resources, placement facilities, and financial agencies to meet patient and family needs.
• Share pertinent information with healthcare team members to develop comprehensive and appropriate discharge plans.
• Comply with policies that protect patients’ rights to considerate care, personal dignity, and respect for their cultural, psychosocial, and spiritual values and preferences.
• Complete assigned goals.
• Perform other duties as assigned.
