About The Position

The Social Worker is responsible for assessing, planning, implementing, coordinating, monitoring, and evaluating the patient’s plan of care from admission through post-discharge. Using critical-thinking skills and case-management principles, the Social Worker coordinates and implements discharge plans that meet patients’ needs and support seamless, effective, and efficient transitions of care across the continuum.

Requirements

Current District of Columbia social work licensure (LGSW, LCSW, or LICSW).
Master’s degree in social work.
Demonstrated ability to arrive on time, limit absences, provide adequate notice to a supervisor for time off, and take breaks within prescribed times.
Complete mandatory training, testing, and competencies on time, including competency training, Services Excellence Training, and an annual physical.
Effective oral and written communication and interpersonal skills.
Knowledge of government and third-party reimbursement systems and contracts.
Strong organizational and problem-solving skills.
Ability to maintain confidentiality.
Ability to adapt to multiple and changing priorities.

Nice To Haves

Three years’ experience in acute care or another related healthcare field.
Case Management Certification.
Previous experience in case management and/or discharge planning.
