About The Position

The Senior Patient Navigator provides support and guidance throughout the patient’s care continuum by helping patients “navigate” through the maze of doctors’ offices, clinics, hospitals, outpatient centers, insurance and payment systems, patient-support organizations, and other components of the healthcare system. The Senior Patient Navigator also acts as a resource for lower-level Patient Navigators to ensure the timely delivery of quality care and patient satisfaction.

Requirements

  • High School diploma or equivalent
  • Minimum 4 years of relevant work experience
  • Ability to communicate effectively in both oral and written form.
  • Ability to handle difficult and stressful situations with professional composure.
  • Ability to maintain effective interpersonal relationships.
  • Ability to understand and follow instructions.
  • Ability to process and handle confidential information with discretion.
  • Must be able to route communications to the appropriate persons(s)
  • Must have strong computer skills
  • Must be able to be redirected as needed for cross coverage and emergent tasks.
  • Make sure lab orders are correct
  • Provide insurance and encounter orders to labs as needed
  • Upload test results to patient record and notify provider and/or counselor of test results ready to review.
  • Communicates with leadership if closures will affect patients
  • Communicates with providers when cross coverage is needed due to the number of patients who will be affected

Responsibilities

  • Maintains a working knowledge of medical symptoms, signs, and anatomical systems to identify and differentiate type and urgency of medical need.
  • Conducts patient evaluations to obtain information necessary for the development of an effective treatment plan.
  • Ensures patients have access to insurance and social service programs for which they are eligible.
  • Maintains knowledge of insurance referral requirements to ensure access based on third party reimbursement criteria.
  • Provides patient reminders of upcoming appointments and acts as liaison between patients and providers.
  • Provides patients with all required information regarding appointments and payment policies (i.e., parking, cash policies, anticipated charges, required ancillary services, cancellation policy).
  • Identifies and resolves real or perceived barriers to care.
  • Intervenes as liaison/advocate for patients, physicians, and staff in facilitating ease of care. Assists in identifying trouble spots and problem patterns in the provision of care.
  • Educates and counsels patients on issues related to their health and life situations.
  • Maintains current knowledge of community health, welfare resources, and any other programs available to patients.
  • Assists in building partnerships with local community physicians and referral sources to the UHealth systems.
  • Assists lower-level Patient Navigators as needed.
  • Adheres to University and unit-level policies and procedures and safeguards University assets.
  • Make sure visits that Central Insurance Verification clears patient visits. (Navigator will only intercede with Medicare and Medicaid patients seen only by counselor)
  • Ensure that all patients have completed the Progeny questionnaire prior to visit or have them rescheduled either by Navigator or Genetic Counselor Assistant.
  • Sent kits out to labs for test orders
  • Minimize available open slots – by reaching out to patients and offer them earlier visit and/or reaching out to Central scheduling to make sure that the team knows all open locations and days.
  • Manage DEP templates for both Cancer Genetics counselors and MDs who are seeing Cancer patients.
  • Close and open schedules for providers based on out of office requests
  • Run reports to give number of patients seen each month by provider, location.
  • Run reports on a monthly basis to give number of patients seen with and without coverage, & with and without payments both self-pay and from insurance
  • Manage Progeny website – Ambry Genetics pedigree system. Managing includes:
  • Giving appropriate access to staff as needed.
  • Troubleshoot issues with logging onto site
  • Working with Progeny help team to resolve any issues that need their intervention.
