Hospice of the East Bay (HEB) is a community-based nonprofit hospice and palliative care organization that has been serving patients and families with compassion and dignity for nearly 50 years. We provide hospice and palliative care in patients' homes and in our six-bed inpatient hospice facility, ensuring every patient receives exceptional care wherever they are most comfortable. Our interdisciplinary team works together to support not only our patients but also their loved ones, offering bereavement services for a minimum of one year after a loss. Under the direction of the attending physician and Medical Director (or Hospice Physician, Nurse Practitioner) and in conjunction with the interdisciplinary group, the main role of a Registered Nurse (RN) is providing direct nursing care and coordinating related assistance for patients and families. Although the RN reports to a primary manager, HEB expects the RN may work in more than one program or department. Assignments are given that best meets the needs of our patients. After Hours: Reporting to the Team Manager, the RN is responsible for making home visits, identifying, and coordinating patient/family care needs during weeknights and weekends. Participate in Triage Nurse Rotation in the absence of Lead Triage Nurse.
