About The Position

AHN Healthcare@Home is hiring a Registered Nurse Transitional Nurse Liaison for a hospice position serving patients in the Pittsburgh, Pennsylvania area. The role provides compassionate, patient-centered end-of-life care in collaboration with an interdisciplinary team, with a focus on comfort, dignity, and quality of life. The position is field-based and requires travel to patient homes within the assigned territory. It is scheduled for 32 hours per week and is eligible for an $8,000 first-year success bonus tied to key achievements.

Requirements

• Active Registered Nurse (RN) license in the state where the employee works.
• At least two years of RN experience.
• Clinical expertise to deliver quality patient care.
• Valid driver’s license.
• Ability to spend approximately 20% of the workday driving to and from patient locations.
• Successful completion of a pre-employment drug screen.

• Hospice experience is preferred.

Responsibilities

• Deliver patient-centered care in a hospice setting in collaboration with an interdisciplinary team.
• Conduct comprehensive hospice admission assessments, including clinical evaluations and medication reconciliation, and develop individualized plans of care aligned with physician orders.
• Educate and guide patients and families through hospice admission, explain the hospice philosophy, set expectations, and support the transition to end-of-life care.
• Use clinical judgment and critical thinking to facilitate timely admissions and address immediate needs.
• Maintain accurate, thorough, and compliant EMR documentation.
• Initiate evaluations by other disciplines when indicated.
• Work independently while collaborating with the care team.
• Travel to patient homes within the assigned territory.
