About The Position

Residential Home Health and Hospice is seeking a Home Health Transitional Nurse Liaison to join its Illinois team and help provide excellent care to patients and caregivers. The organization provides services across the continuum, from rehabilitative home health to end-of-life hospice care, and offers employees training, resources, and support to deliver care to patients and caregivers. The liaison obtains patient information, introduces the company and its services, coordinates start-of-care visits, and supports safe transitions from facilities to home. The role also includes patient and community education, coordinating requested services and equipment, and collaborating with patients, caregivers, physicians, referral sources, and facility teams.

Requirements

RN licensure in good standing.
At least one year of experience in an acute care setting.
At least one year of experience in a customer service capacity.
Commitment to providing the highest level of customer service to referral sources.
Ability to multitask.

Responsibilities

Obtain complete and accurate patient demographic information, medical history, diagnosis for care, and primary care physician information.
Introduce the company to patients and caregivers and explain the scope of services and skilled services requested.
Coordinate start-of-care visits.
Order requested durable medical equipment (DME), infusion services, and wound care.
Provide community education at physician offices and assisted living facilities, including educational in-services, blood pressure clinics, and health fairs, as requested.
Educate patients at bedside.
Arrange post-discharge physician follow-up appointments and share important discharge information with the primary care physician.
Attend discharge planning meetings as requested and work with facility teams to support a safe and successful discharge home.
Determine the appropriateness of skilled home care services.
