About The Position

At Elara Caring, the Transitional Liaison Registered Nurse helps identify patients who may qualify for hospice care and supports their transition to hospice services. The role follows hospice referral, admission, and documentation processes; assesses patients’ physical, emotional, social, and spiritual needs; and coordinates transitions and services with care teams. The nurse meets with patients and caregivers to discuss care goals and complete initial assessments, explains hospice services to support informed decisions, and educates staff and referral partners about hospice eligibility and the transition process.

Requirements

• Current state license as a Registered Nurse.
• Graduate of an accredited certificate, diploma, associate, or baccalaureate school of nursing.
• At least 3 years of experience in a clinical care setting.

Nice To Haves

• Experience within Home Health or Hospice is preferred.
