About The Position

By The Bay Health is a non-profit established in 1975 that provides Skilled Home Health Care, Palliative Care, Adult Hospice Care, and Pediatric Care. Its team-based approach addresses practical, social, emotional, and spiritual aspects of care to maximize quality of life for patients, caregivers, and families. The After-Hours Nurse provides skilled nursing and hospice care to maximize patients’ comfort and health. This full-time role is based in Larkspur, California, with coverage in Marin County and additional coverage in Sonoma and Napa Counties as needed. The schedule is Thursday and Friday from 3:30 p.m. to 12:00 a.m., and Saturday and Sunday from 9:00 a.m. to 5:30 p.m. The position offers competitive pay, generous benefits, and a supportive work environment. Requirements

One year of experience as a professional nurse in the past three years, or a Bachelor’s degree in Nursing from an accredited School of Nursing.
Current license as a registered professional nurse in the State of California.

Nice To Haves

Experience in acute care, home health, and/or a hospice setting is preferred.
Public Health Nursing certification is highly desirable.
HPNA certification is preferred.

Responsibilities

Provide continuing care to patients and families as requested and in compliance with agency policies and procedures and applicable federal and state regulations.
Under the guidance of the After-Hours Nurse, assess, plan, implement, and evaluate nursing services for patients and families consistent with the plan of care.
Communicate with attending physicians and other community resources to report changes and coordinate optimal care and use of resources for patients and families.
Maintain regular communication with Team Leaders regarding patient and family status and unusual or potentially problematic issues.
