About The Position

Heart to Heart Hospice provides compassionate care to patients with life-limiting illnesses and their loved ones. The RN, After Hours provides on-call nursing care, including home visits for assessment, treatment, and support. Visits may involve pronouncing a patient’s death, managing a crisis, or completing an admission. The nurse plans and coordinates comprehensive services for patients and families, documents care and progress, communicates changes and outcomes to physicians and other personnel, and ensures compliance with applicable laws, regulations, and agency policies.

Requirements

Graduate of an accredited diploma, associate, or baccalaureate school of nursing.
Current state Registered Nurse license.
Current driver’s license, reliable transportation, and ongoing valid and current auto liability insurance.

Nice To Haves

Two years of RN experience in a healthcare or clinical setting preferred.

Responsibilities

Provide home visits as needed for patient assessment, treatment, and support, including death pronouncement, crisis management, and admission.
Plan comprehensive services for patients and families; develop or revise nursing care plans and goals with the patient’s physician and the IDG.
Use excellent customer care skills with internal and external customers, including proper phone etiquette.
Prepare clinical and progress notes demonstrating progress toward established goals.
Inform physicians and other personnel of changes in patient needs and intervention outcomes, and evaluate patient and family responses to care.
Submit all documentation in a timely manner.
Perform other duties as required.
Ensure compliance with local, state, and federal laws, Medicare regulations, and established personnel policies and procedures.
