About The Position

The Registered Nurse On Call provides home visits as needed for patient assessment, treatment, and support. Visits may involve pronouncing a patient’s death, crisis management, and admission. The nurse plans comprehensive services for patients and their families and develops or revises nursing care plans and goals in conjunction with the patient’s physician and the IDG. The role also includes communicating changes in patient needs and outcomes, documenting care and progress, and ensuring compliance with applicable laws, regulations, and policies.

Requirements

Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
Current State Registered Nurse License.
Current driver’s license and reliable transportation.
Provide ongoing valid and current auto liability insurance.

Nice To Haves

Two years of experience as an RN, preferably in a healthcare or clinical setting.

Responsibilities

Provide home visits as needed for patient assessment, treatment, and support, including visits for pronouncement of patient death, crisis management, and admission.
Plan comprehensive services for patients and their families.
Develop and/or revise nursing care plans, including goals, in conjunction with the patient’s physician and IDG.
Use excellent customer care skills with internal and external customers, including proper phone etiquette.
Prepare clinical and progress notes that demonstrate progress toward established goals.
Inform the physician and other personnel of changes in the patient’s needs and outcomes of interventions.
Evaluate patient and family responses to care.
Submit all documentation in a timely manner.
Perform other duties as required.
Ensure overall compliance with local, state, and federal laws, Medicare regulations, and established personnel policies and procedures.
