About The Position

Elara Caring is hiring a full-time Hospice RN for an after-hours, on-call role on a 7-on/7-off schedule. The nurse provides hospice care in patients’ homes and assigned care settings, assessing patient and family needs, monitoring changes in condition, and supporting the interdisciplinary Plan of Care. The role also includes educating patients, families, caregivers, and facility staff about hospice care, pain management, and symptom management, as well as providing compassionate support at the time of death and completing bereavement assessments.

Requirements

• Current state license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing.
• At least 1 year of experience as a nurse in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Valid driver's license and acceptable driving record, or reliable transportation to travel between patient homes and assigned work locations.

Nice To Haves

• Home Health experience is preferred, but not required.
• Experience in a hospice or home health environment is preferred.

Responsibilities

• Assess hospice patients and families to identify physical, psychological, social, spiritual, and environmental needs.
• Monitor patient condition and communicate significant changes, concerns, or safety issues to the physician and care team.
• Educate and support patients, families, caregivers, and facility staff in following the interdisciplinary Plan of Care.
• Educate staff in nursing homes, hospitals, assisted living facilities, and other care settings about hospice philosophy, pain management, and symptom management.
• Provide compassionate support at the time of death and complete bereavement assessments for families and caregivers.
