About The Position

Phoenix Home Care and Hospice is seeking a compassionate Registered Nurse Home Health Case Manager to manage patient care in Kansas City, Missouri, and surrounding areas. The role involves managing an individual caseload, supervising LPN visits, conducting varied home health visits, educating patients, and coordinating care with physicians and an interdisciplinary therapy team. Nurses manage their own schedules and use Roger, an AI-powered charting assistant, to complete documentation in real time during visits. Requirements

Active Missouri or Compact RN license.
Valid Missouri driver's license.
Strong clinical skills in physical assessment and diagnostic interpretation.
Ability to organize, work independently, and manage an individual caseload.
Compassionate communication skills with patients, families, and physicians.

Home health experience is strongly preferred.

Responsibilities

Manage an individual caseload, including supervision of LPN visits, and manage your own schedule.
Conduct varied weekly visits, including routine care, evaluations, and starts of care.
Educate patients about health conditions and care, including CHF, COPD, diabetes, wound care, and medication management.
Collaborate with physicians to obtain orders and develop diagnosis-driven plans of care.
Work with the therapy team, including physical therapy, occupational therapy, and speech therapy.
Manage patient care effectively and collaborate with interdisciplinary teams.
Ensure compliance with regulatory standards.
