About The Position

The Home Health Case Manager provides hands-on patient care in patients’ homes and coordinates care with other healthcare providers and health insurance companies. The role oversees visiting nurses and develops personalized care plans aligned with physician orders, using thorough initial assessments, physical assessment skills, and diagnostic interpretation. The case manager collaborates with nurses and Physical Therapy, Speech Therapy, and Occupational Therapy professionals. Nurses complete charting during visits with support from Roger’s AI-powered tools.

Requirements

• Missouri or Compact Registered Nurse license.
• Driver’s license.
• Vehicle with current auto insurance.
• Strong background in physical assessment, diagnostic interpretation, and effective communication.

Nice To Haves

• Home health or critical care experience preferred.

Responsibilities

• Manage a caseload, including supervising LPN visits.
• Conduct routine visits, evaluations, and starts of care, with ongoing day-to-day patient interaction.
• Educate patients about health conditions and care, including CHF, COPD, diabetes, wound care, and medication management.
• Communicate with physicians to obtain orders and write diagnosis-driven plans of care.
• Coordinate with other healthcare providers and health insurance companies, and oversee visiting nurses.
• Work alongside the Physical Therapy, Occupational Therapy, and Speech Therapy team.
• Complete charting during patient visits with support from Roger’s AI-powered tools.
