About The Position
Phoenix Home Care and Hospice is hiring a full-time Home Health RN Case Manager in St. Louis, Missouri. The role offers a Monday–Friday schedule, with on-call approximately once a month, including admissions. Nurses have autonomy to build their daily visit schedules and manage a caseload using a pay-per-visit compensation model. The stated base pay is $78,000–$88,000+, with many nurses earning above that range through additional visits. The position includes 4–6 weeks of paid training with guaranteed pay while ramping up. The company offers charting support through its AI tool, Roger, and invites candidates to complete a ride-along with a nurse before signing. Mileage is reimbursed for every visit, with additional out-of-town visit pay for qualifying trips beyond the nurse's territory.
Requirements
- Missouri or Compact RN license.
- Valid driver's license.
- Strong organizational, clinical, and communication skills.
Responsibilities
- Manage a caseload of home health patients.
- Supervise LPN visits.
- Coordinate care with Physical Therapy, Occupational Therapy, and Speech Therapy teams.
- Conduct patient assessments.
- Develop personalized care plans aligned with physician orders.
- Support patients managing conditions such as CHF, COPD, diabetes, wounds, and medication regimens.
