About The Position

Brookhaven Hospice is seeking a self-motivated and passionate Hospice RN Clinical Liaison. The role primarily conducts outreach at hospitals and medical facilities to identify referral sources and maintain relationships that increase hospice census and preference for hospice services. The liaison uses clinical knowledge, empathetic communication, and marketing skills to educate referral sources, families, and prospective patients about hospice services, and serves as a hospice resource to referral sources to support excellent patient and customer service. The territory covers Bedford and Manchester Hospitals.

Requirements

Hold a current, unencumbered registered nurse license.
Have a minimum of one year of sales or marketing experience in healthcare.
Be skilled at establishing and maintaining working relationships with key staff, marketing contacts, and clinical staff to ensure understanding of hospice services.
Demonstrate the ability to generate and maintain relationships with new and existing accounts, including physicians, healthcare professionals, and community agencies.
Be willing to drive and have reliable transportation, a valid driver's license, and auto insurance.

Nice To Haves

Bachelor's degree in nursing or healthcare.
One to two years of hospice or palliative care marketing experience.

Responsibilities

Identify primary decision makers and proactively build positive working relationships with members of the medical community and partners.
Promote and position hospice patient care services through regular visits that reflect the hospice's quality of care and capabilities.
Build the hospice brand as the preferred provider in the service area and help achieve agency growth and census-management goals.
Make planned calls to secure referrals, build census, and meet established goals with referring physicians, facilities, hospital and skilled-nursing-facility case managers, discharge planners, social workers, and other healthcare providers.
Assist the hospice in determining patient eligibility based on hospice knowledge and training, as needed.
Conduct initial meetings with patients and families to explain hospice services and philosophy and obtain consents for care.
