About The Position

Brookhaven Hospice is seeking a self-motivated, passionate Hospice RN Clinical Liaison. This role conducts outreach at hospitals and medical facilities to identify referral sources and maintain relationships that support hospice census growth and preference for hospice services. The liaison uses nursing knowledge, empathetic communication, and marketing skills to educate referral sources, families, and prospective patients about hospice care, and serves as a hospice resource to referral partners to support patient and customer service. The territory includes Bedford and Manchester Hospitals.

Requirements

• Current, unencumbered registered nurse license.
• At least one year of sales or marketing experience in healthcare.
• Skilled at establishing and maintaining working relationships with key staff, marketing contacts, and clinical staff to ensure understanding of hospice services.
• Ability to generate and maintain relationships with new and existing accounts, including physicians, healthcare professionals, and community agencies.
• Willingness to drive, with reliable transportation, a valid driver’s license, and auto insurance.

Nice To Haves

• Bachelor’s degree in nursing or healthcare.
• One to two years of hospice or palliative care marketing experience.

Responsibilities

• Identify primary decision makers and proactively build positive working relationships with members of the medical community and partners.
• Promote hospice patient care services through regular visits that reflect the quality of care and the hospice’s capabilities.
• Build the hospice brand as the preferred provider in the service area and work toward agency growth and census goals.
• Make regular, planned calls to obtain referrals and build census among referring physicians, facilities, hospital and skilled nursing facility case managers, discharge planners, social workers, and other healthcare providers.
• Help determine patient eligibility when appropriate, using hospice knowledge and training.
• Conduct initial meetings with patients and families to explain hospice services and philosophy and obtain consents for care.
