RN Field Case Manager
About The Position
AccordCare Home Health is seeking an RN Field Case Manager to join its Atlanta office. The position is primarily field-based, with occasional office time as needed to support high-quality client care. The role works with caregivers, clients, referral sources, and the office team to coordinate care and create positive client experiences in a supportive, team-oriented environment. The RN Field Case Manager manages home care cases, serves as a liaison among clients, caregivers, and referral sources, and assists with office operations such as scheduling, authorizations, and client communications. The position also includes aide orientations and training, supervisory visits, compliance monitoring, and collaboration with leadership to improve client satisfaction and caregiver performance.
Requirements
- Current RN license in the State of Georgia.
- Excellent communication, leadership, and organizational skills.
- Positive, professional attitude and a strong commitment to quality care.
- Reliable transportation and a valid driver’s license for occasional field visits.
Nice To Haves
- One or more years of experience in home care or community health is preferred.
- Familiarity with AlayaCare software.
Responsibilities
- Manage and coordinate home care cases to ensure clients receive exceptional service and support.
- Serve as the main liaison between clients, caregivers, and referral sources.
- Oversee daily office operations, including assisting with scheduling, authorizations, and client communications.
- Conduct aide orientations, trainings, and supervisory visits to promote quality and consistency in care.
- Monitor compliance with AccordCare policies and state and federal regulations.
- Collaborate with leadership to continuously improve client satisfaction and caregiver performance.
