About The Position

Trustbridge Hospice, a member of Empath Health, is seeking a caring and motivated Hospice Registered Nurse (RN) Case Manager to provide patient-centered hospice care in the northeast Broward County area. The role focuses on coordinating care and supporting patients and families throughout the hospice journey, helping ensure comfort, dignity, and quality of life. The schedule is Monday through Friday, 8:30 a.m. to 5:00 p.m., with a weekend requirement.

Requirements

Current Registered Nurse license in the State of Florida.
Current Basic Life Support (BLS) certification.
Understanding of and ability to apply hospice nursing characteristics, including case management, crisis intervention, communication, teaching, thanatology skills, and needs.
Basic computer skills.
Adequate and reliable transportation, adequate insurance, and a valid Florida driver’s license.

Nice To Haves

Bachelor of Science in Nursing (BSN).
Hospice certification.
One year of experience in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing, or hospice nursing.
Experience with electronic medical record (EMR) software.

Responsibilities

Advocate for patients and families, ensuring their needs are recognized and addressed.
Coordinate comprehensive care under the direction of the primary physician and in collaboration with the interdisciplinary team.
Develop and implement individualized plans of care as part of the Interdisciplinary Group (IDG).
Monitor patient conditions, provide interventions, and deliver education to enhance comfort and quality of life.
Address concerns promptly and thoughtfully to ensure a high level of satisfaction and trust.
