About The Position

Home Health Advantage, Inc., an established Medicare-certified home health agency, is seeking an energetic and highly personable Registered Nurse (RN) to join our team as an RN Clinical Liaison. This is a unique role for an RN who enjoys working with people, building professional relationships, and helping patients transition safely from the hospital to home. This is not a traditional bedside or field nursing position.

Requirements

Current Illinois RN license.
Exceptional interpersonal and communication skills.
Excellent computer and technology skills.
Professional, energetic, confident, and approachable personality.
Strong organizational skills and attention to detail.
Ability to communicate comfortably with patients, families, physicians, case managers, and other healthcare professionals.

Nice To Haves

Home health, hospital, case management, intake, or care coordination experience.
Familiarity with Medicare home health services.

Responsibilities

Meet with prospective patients and families in the hospital prior to discharge.
Explain home health services and help patients understand what to expect after returning home.
Coordinate referrals and transitions of care with hospital case managers, discharge planners, physicians, and our internal clinical team.
Maintain excellent relationships with hospital staff and referral partners.
Communicate referral information accurately and promptly to our intake and clinical teams.
Follow patients through the discharge and admission process to help ensure a smooth transition home.
Utilize electronic medical records, referral platforms, email, and other technology throughout the workday.
Represent Home Health Advantage professionally within the hospital environment.
