PRN On-Call RN Case Manager
About The Position
Lifted Hospice seeks an On-Call Registered Nurse Case Manager to provide personalized hospice nursing care to patients in their homes or other care settings. The role combines clinical care and compassionate support for patients and families facing end-of-life. The RN manages symptoms, guides care, and collaborates with the interdisciplinary team to create and follow care plans that reflect patient goals and meet clinical, ethical, and emotional care standards. The RN also educates and supports families and caregivers so they can provide comfort and feel prepared during this time. The position is primarily field-based, with care delivered in private residences, memory care facilities, and other long-term care facilities. The RN meets with the local interdisciplinary team bi-weekly to align patient care plans. On-call coverage is bi-weekly until further notice: Monday through Thursday from 5 p.m. to 8:30 a.m., and Friday from 5 p.m. through Monday at 8:30 a.m.
Requirements
- Graduate of an accredited Diploma, Associate, or Bachelor’s nursing program.
- Active state nursing license and clinical skills that meet current standards of nursing practice.
- Experience as a registered nurse in a clinical care setting.
- Excellent interpersonal skills.
- Ability to read, write, and comprehend English.
- Current driver’s license, reliable transportation, and valid auto liability insurance.

Responsibilities
- Complete thorough assessments based on each patient’s physical, emotional, and clinical needs.
- Provide skilled hospice nursing care to manage pain and symptoms and improve quality of life.
- Collaborate with the care team, patients, and loved ones to create and follow a unified care plan.
- Document care and updates accurately in accordance with applicable standards and requirements.
- Educate and encourage caregivers, provide emotional support, and help families feel prepared and confident.
- Meet with the local interdisciplinary team bi-weekly to align patient care plans.
