About The Position
PruittHealth Hospice is hiring a Registered Nurse Case Manager to provide hospice care in patients’ homes or care facilities. The position is for weeknight or weekend on-call work. The role supports continuity of patient care and requires providing evaluations, assessments, and treatments in accordance with policy and each patient’s individualized care plan, while maintaining timely and accurate documentation. Requirements
- Current, active, and unrestricted Registered Nurse (RN) licensure in the state of practice.
Responsibilities
- Complete evaluations, assessments, and treatments in accordance with policy and the individualized care plan.
- Complete all documentation and records in a timely and accurate manner.
- Follow established policies and procedures related to the scope of practice.
- Maintain continuity of patient care in a hospice setting.
