PRN On-Call RN Case Manager
About The Position
Lifted Hospice is seeking an On-Call Registered Nurse (RN) Case Manager to provide personalized hospice nursing care in patients’ homes and other care settings. The role combines clinical expertise with compassionate support for patients and families facing end-of-life. The RN manages symptoms, guides care, and helps families feel informed, prepared, and supported. The RN collaborates with the interdisciplinary team to develop and carry out care plans that reflect patient goals and meet clinical, ethical, and emotional care standards. The position provides care in private residences, memory care facilities, and other long-term care facilities, and includes bi-weekly meetings with local team members to align care plans. On-call coverage follows a bi-weekly schedule until further notice: Monday through Thursday, 5 p.m. to 8:30 a.m., and Friday, 5 p.m. through Monday, 8:30 a.m.
Requirements
- Graduate of an accredited Diploma, Associate, or Bachelor’s nursing program.
- Active state nursing license.
- Clinical skills that meet current standards of nursing practice.
- Experience as a registered nurse in a clinical care setting.
- Excellent interpersonal skills.
- Ability to read, write, and comprehend English.
- Current driver’s license, reliable transportation, and valid auto liability insurance.

Responsibilities
- Complete thorough assessments of patients’ physical, emotional, and clinical needs to guide care.
- Deliver skilled hospice nursing care to manage pain and symptoms and improve quality of life.
- Collaborate with the care team, patients, and their loved ones to create and follow a unified care plan.
- Document care and updates accurately in accordance with applicable standards and requirements.
- Educate and encourage caregivers, provide emotional support, and help families feel prepared and confident.
- Meet bi-weekly with the local interdisciplinary team to align patient care plans.
