About The Position

Careforth is seeking an RN Care Manager to provide personalized care management and support to caregivers and their loved ones. The role combines in-person home visits, remote coaching, and use of a digital collaboration app. The RN assesses needs, develops and follows individualized care plans, supports caregivers, and helps ensure exceptional consumer care. The position offers a flexible schedule, with no night shifts, weekends, or on-call duties required. The position involves travel within an assigned geographic area to conduct home visits. Requirements

Hold an active Registered Nurse (RN) license in the state of employment.
Have a minimum of two years of recent experience providing direct care to elders or adults with disabilities.
Be able to travel, usually by automobile, within the assigned geographic area; a valid driver’s license and automobile insurance are required.
Have experience in case management, care planning, and assessment for cases including eldercare, people with disabilities, and other complex medical needs in a health care setting.
Be proficient in common software and web-based applications, such as MS Office and case management systems.
Meet any additional state-specific qualifications that may be required.

Responsibilities

Conduct home visits in the assigned territory to assess home safety, gather updates on health status, and provide clinical coaching and support, including medication review.
Provide ongoing caregiver coaching and support through the app while following an established care plan; refer caregivers to resources and reinforce best practices.
Develop trusting relationships with caregivers to influence change and reduce caregiver burnout.
Document and review records for accuracy and timeliness and ensure compliance with regulatory and accreditation standards.
Perform other projects and duties as assigned.
