About The Position

This position provides direct support to individuals and staff. The Community Specialist leads a small staff team supporting and advocating for clients as they work toward maximum independence. The role helps individuals achieve self-selected goals, build independence, feel supported in daily living, and create safe and healthy lives. It also involves recommending strategies as part of the Individual Support Team and advocating for quality care inside and outside the agency. The Community Specialist builds, engages, and supervises staff assigned to individuals to ensure continued quality service. The position may also assist in the Day Program and Residential home when needed and participates in the Community Specialist team’s weekend and evening emergent coverage.

Requirements

Able to work weekends and evenings as part of a regional team to cover emergent needs.
Able to pass TB and drug screening as well as a criminal history check.
Must have reliable transportation.
Must be able to sit or stand for long periods of time.
Must be able to grasp the equivalent of a 12 oz. can.
Must be able to lift up to 50 lbs.
High school diploma or GED.

Responsibilities

Support individuals in daily living to promote safe and healthy lives.
Recommend strategies as part of the Individual Support Team to help individuals achieve goals and greater independence.
Advocate for assigned individuals to ensure they receive quality care within and outside the agency.
Build, engage, and supervise staff working with assigned individuals to ensure continued quality service.
Assist in the Day Program and Residential home when needed.
Provide weekend and evening emergent coverage as part of a regional team.
