Residential Coordinator-Northern Colorado

Sample Supports•Fort Collins, CO

About The Position

Sample Supports is seeking a Residential Coordinator to support adults with intellectual and developmental disabilities living in host homes, with family caregivers, and in supported apartment settings. The role combines care coordination, provider oversight, advocacy, compliance, and relationship-building. Residential Coordinators manage an assigned caseload, monitor service quality and safety, support residential providers, and coordinate with each individual’s interdisciplinary team. Three levels are available based on experience, demonstrated competency, and leadership responsibilities; qualified applicants will be considered for the level that best matches their background. Candidates must complete the Culture Index Survey to be considered. The survey is described as a tool for building balanced teams and matching people with roles where they can thrive, rather than a test with right or wrong answers. The position requires travel among residential settings, appointments, meetings, and Sample Supports locations, and may involve transporting individuals. Coordinators participate in a scheduled or assigned 24/7 on-call rotation and may provide backup direct care, transportation, or shift coverage. They must complete required training and certifications, including CPR, First Aid, Medication Administration, and NVCI, and must be prepared to perform the role’s physical responsibilities and respond professionally to emergencies and behavioral escalations. The coordinator works independently while connected to a collaborative team and may support the Residential Manager and serve as a resource for other coordinators. The role has three levels: Residential Coordinator I is an entry-level coordinator; Residential Coordinator II independently manages a full caseload; and Residential Coordinator III has advanced residential, compliance, and leadership experience. Benefit eligibility may depend on employment status, tenure, hours worked, and applicable plan requirements.
