About The Position

Rivera Mansions LLC is seeking an experienced, energetic, and compassionate Resident Care Coordinator (RCC) to manage and support direct care staff. This role offers significant growth opportunities with a direct path to administrator positions. The RCC will lead a team of caregivers and med-tech staff alongside the community Registered Nurse and Facility Administrator. The ideal candidate is professional, compassionate about the elderly, a team player, an excellent communicator, and possesses the managerial courage to handle people effectively.

Requirements

  • A Bachelor’s degree in Healthcare Management, or similar program is preferred.
  • LPN is preferred. If LPN, must possess a current, unencumbered, active license to practice as an LPN in this state.
  • At least Two (2) or more years of experience in a supervisory/managerial capacity with progressive levels of responsibility.
  • Knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines pertaining to residential care facilities.
  • Experience with Electronic Medical Records and computer documentation systems.
  • Experience and knowledge of current federal and state healthcare/assisted living/residential care facility rules and practices.
  • Working knowledge of Microsoft 365 office technology and software (MS SharePoint, MS Teams, MS Office apps: Word, Excel, Outlook, etc.).
  • Excellent writing and communication skills.
  • Ability to lift up to 30 pounds, aid in resident transfers, walk/stand 80% or more of shifts, and respond to resident and emergency needs quickly, with or without reasonable accommodation.
  • All new staff hired must provide acceptable evidence of vaccination or become vaccinated for COVID-19 immediately upon hire as a condition of employment.

Responsibilities

  • Develops, executes, and maintains resident care plans that meet individual needs and physician recommendations.
  • Works with nurses, med-aides, and caregivers to build relationships and strong communication with residents, families, and associates.
  • Maintains a documented system of operations policies and procedures, ensuring all facility practices are carried out ethically. Makes recommendations for improvements in procedures, policies, methods, education, reference material, and resident care/nursing practices.
  • Participates in the manager on duty and clinical on-call rotation, including weekend on-call duties and covering clinical shifts as needed.
  • Assists management with human resource functions, including organizational structure design, recruitment, training, and staff performance evaluation.
  • Oversees day-to-day operational functions including direct care staff performance, staff scheduling, medication administration (ordering, receiving, documentation, storage, and review), daily rounds to ensure work assignments are performed, and coordination of resident appointments and services.
  • Serves as a liaison between residents and physicians, home health care agencies, pharmacies, and ancillary services, and maintains contact with hospitals for transferred residents.
  • Collaborates with nutritionist/cook/kitchen staff to ensure residents' nutritional needs are met according to service plans and ensures quality assurance of the dining experience.
  • Plans resident activities as required by State guidelines.
  • Orders and tracks inventory of supplies, including equipment, diabetic supplies, and incontinence supplies.
