About The Position

HCA Houston Healthcare is seeking a PRN Clinical Rehab Liaison to support facilities and the market network across the HCA Healthcare Gulf Coast Division. The liaison serves as a clinical consultant and market specialist, bridging healthcare facilities, referral sources, and potential patients to support rehabilitation referrals and admissions. The role conducts pre-admission screenings, evaluates medical necessity under inpatient rehabilitation criteria, builds referral partnerships, and helps ensure smooth transitions into rehabilitation programs.

The position is open to experienced Registered Nurses, Licensed Physical Therapists, Registered Occupational Therapists, and Speech-Language Pathologists. As an as-needed division-wide resource, the liaison provides coverage across multiple campuses, including during peak census periods, and supports business development throughout the Gulf Coast market.

Requirements

• Graduate of an accredited school in a clinical specialty.
• Current Texas license in a clinical health care specialty, such as Registered Nurse, Licensed Physical Therapist, Registered Occupational Therapist, or Speech-Language Pathologist/Therapist.
• Current BLS certification.
• Minimum of 2 years of recent clinical care experience.

Responsibilities

• Serve as an inpatient rehabilitation expert between internal and external referral sources and inpatient rehabilitation operations.
• Establish relationships with case managers, physicians, payors, and other referral sources.
• Assist with and coordinate intake and pre-admission screening.
• Evaluate patients for medical necessity under inpatient rehabilitation criteria and document evidence of admission criteria on pre-screen evaluations.
• Act as the primary contact for case management and referring physicians.
• Provide referral sources with appropriate and timely follow-up communications about their patients and hospital programs.
• Assist in developing the annual strategic plan for the Rehab Unit or Hospital, including community outreach programming and raising awareness of available services.
• Promote rehabilitation programs and outcomes, streamline the referral-to-admission process, and help overcome barriers to admission.
