About The Position

As a Home Health Registered Nurse, you’ll deliver skilled nursing care in patients’ homes, create individualized care plans, administer medications, educate patients and families, and support recovery and independence. The role offers autonomous practice and meaningful one-to-one patient relationships for nurses experienced in home health or transitioning from hospital, acute care, or skilled nursing.

The schedule is three 12-hour shifts, Thursday–Saturday and/or Saturday–Monday. There are no on-call requirements. The position uses Epic EHR for documentation and care coordination. Comprehensive onboarding and an individualized orientation program are provided.

Requirements

• Graduate of an approved nursing education program.
• Current Registered Nurse licensure in the State of Michigan.
• At least one year of professional registered nursing experience.
• Strong computer and technology skills.
• Current driver’s license and reliable transportation.

Nice To Haves

• Bachelor of Science in Nursing (BSN) preferred.
• Medical-surgical experience preferred.
• Home health or hospice experience is a plus.
