As a Homecare RN, you will deliver primary nursing care as ordered by the physician, develop individualized care plans in collaboration with the Population Health Case Manager, and ensure exceptional patient outcomes and satisfaction. Coverage regions include Ann Arbor and Ypsilanti.

Requirements

• Graduate of an accredited nursing program.
• Current RN licensure in the state of practice.
• At least one year of professional nursing experience.
• Strong computer and technology skills.
• Current driver’s license and reliable transportation.

Nice To Haves

• Bachelor of Science in Nursing (BSN) preferred.
• Home health experience preferred.

Responsibilities

• Conduct comprehensive patient assessments at admission and at ongoing intervals.
• Develop and implement individualized, assessment-based care plans in partnership with physicians and case managers.
• Deliver therapeutic interventions and education to promote independence and improve health outcomes.
• Monitor and report changes in patient condition, adjusting care plans as needed.
• Use motivational interviewing and health coaching techniques to support patient goals.
• Document care accurately and promptly in compliance with agency policies and regulatory standards.
• Mentor and support team members to ensure quality care delivery.
